What is Penile Fracture and Who is at Risk? 

Penile fracture is a  dangerous problem that can occur during sex, but fortunately, it is fairly uncommon.  

A penile fracture refers to a tear or rupture of the penile sheath that surrounds the penis.  Tunica albuginea surrounds the penis like a sausage casing.   The sheath called tunica albuginea is responsible for maintaining pressure in the penis that allows an erection to occur.  

The most common cause of penile fracture is from an erect penis slipping out from a vagina and hitting the perineum during sex. It appears that woman-on-top is the highest risk position for a penile fracture to occur, but it can occur in any position. During vigorous sexual activity, if the penis slips out and hits something hard, it generates very high torque force on tunica albuginea and can lead it to be torn.  

Symptoms of Penile Fracture

The most common symptom of penile fracture is a ‘popping sound,’ that occurs often but not always, followed by severe pain and loss of erection. “Black-and-blue” hematoma can occur on the penis; some men may also experience bleeding from the tip of the urethra or blood in the urine

Typically, a penile fracture is considered a urological emergency.  Surgical repair of the tear usually results in good outcomes.  If a penile fracture is severe and not treated in a timely manner, it can lead to problems with obtaining or maintaining erections (erectile dysfunction); it may also cause scar formation in the penis and a condition called Peyronie’s disease, which causes curvature and deformity of the penis.

What are the long-term effects of a broken penis?

Penile fracture can lead to impotence, inability to obtain and maintain erections.  If it heals with a scar, it can also lead to penile curvature called Peyronie’s disease.  If a penile fracture is associated with urethral injury, urethral stricture (narrowing) can occur. 

What Exactly Is Breaking Inside Their Penis If It’s Not A Bone?

A sausage-like casing called tunica albuginea holds the penis together. A penile fracture refers to a laceration or tear that occurs in the tunica albuginea.  When that happens, the pressure inside the penis cannot be generated and maintained to get and maintain an erection.  Blood, which is what creates an erection, does not stay ‘inside the penis’ and can leak.  In addition, when the scar heals, it may result in penile curvature.  In about 20% of men, the urethra is also injured from the trauma. Often but not always there is an audible ‘pop’ and sudden onset of penile pain.  Typically the erection is lost.  Sometimes black-and-blue and a hematoma (collection of blood) develops around the site.

Will the Penis be Always Bent or Crooked After Penile Fracture?

If the injury is repaired in a timely fashion, as in immediately within 24 hours of injury, most men heal without significant long-term problems.  If the injury is not repaired in a timely manner, men may develop erectile dysfunction and sometimes Peyronie’s disease resulting in a bent/crooked penis.

How does Penile Fracture Affect Fertility?

Penile fracture does not affect a man’s sperm quality. But if it causes difficulty with erections, it may be difficult to get a partner pregnant through vaginal sex without good erections unless ED is treated or another option is used to get the partner pregnant (intrauterine insemination or IVF).  When properly treated, most men with a history of penile fracture are able to conceive children through vaginal sex.
