What You Need to Know about Treatment of  Penile Torsion

Penile torsion What is Penile Torsion?

Penile torsion refers to the rotation of the penis or corkscrew-like appearance of the penis. Penile curvature may be a manifestation of penile torsion.

Typically, the head of the penis or the glans of the penis is rotated relative to the shaft of the penis. This rotation is usually lifelong and has been present since birth. Often, penile torsion is not noticed until the young man becomes self-aware of the appearance of the penis when it is erect, which typically occurs in adolescence or young adulthood. The penis appears straight and normal when it is not erect. However, occasionally with severe penile torsion, the penis may appear curved even when it is flaccid (non-erect).

Penile torsion is more noticeable and pronounced when there is a full erection. With severe penile torsion, sexual activity may be difficult or impossible. Sometimes urination may be difficult or may require extra steps to point the penis into the toilet.

The severity of penile curvature can only be judged when the penis is fully erect. If the penis is less than fully erect, the severity of penile rotation can be underestimated.

What are the Causes of Penile Torsion

Penile torsion is usually a congenital condition, which means that a man is born with it. The actual problem that causes rotation of the penis is not always easy to establish.

Relative malrotation of corpora cavernosa or corpus spongiosum may lead to penile torsion. Length differences between the two corpora cavernosa as well as the torque caused by variance in density of the connective tissue may also lead to uneven forces applied to the skin of the penis when the penis is erect, which can lead to penile torsion.

Treatment of Penile Torsion

The treatment of penile torsion depends on the underlying cause.

If penile torsion is caused by skin misalignment after circumcision can often be corrected by circumcision revision with penile degloving.

For patients with congenital penile torsion, penile degloving can often help, but additional maneuvers may be necessary, including counter tension, penile plication, and other surgical maneuvers.

Depending on the severity of penile torsion, the procedure can often be performed in our office under local anesthesia.  For more severe cases, the procedure may need to be performed in the hospital or surgical center under general anesthesia.

What to Expect Before and After Penile Torsion Surgery?

Most men are able to go home the same day after penile torsion correction surgery. Most men are able to go back to work at school within 2 to 3 days after the procedure.

At New York Urology Specialists, we advise men who undergo surgery for the correction of penile torsion not to go to the beach or a swimming pool for 2 weeks after the procedure. Men should avoid sexual activity for 4 to 6 weeks after the procedure until the surgeon judges that the healing is complete and that sexual activity can be resumed.  Men can take a shower the next day after the procedure.

Other Causes of Penile Curvature

In addition to penile torsion, penile curvature can also be caused by frenulum breve (short penile frenulum), Peyronie’s disease, epispadias, and a few other less common urological problems.
