Peyronies Treatment: 32-year-old Man Treated at New York Urology Specialists
JM is a 32-year-old man with complaints of pain in the penis that started 3-4 months ago. He noticed a new slight curvature in the penis over the past month. Recently he noticed a lump in the penis. He reports no prior history of penile trauma. He reports no prior treatment. He reports no difficulty with getting and keeping erections.
The patient was evaluated. Blood work revealed normal testosterone, normal cholesterol levels. His blood pressure was within a normal range.
Physical exam revealed a palpable dorsal penile plaque in the mid-to-distal shaft about 1.5 cm long. Images were reviewed which revealed a slight curvature.
Follow-up Appointment – 2 weeks
Penile Doppler ultrasound revealed no vascular insufficiency and no evidence of veno-occlusive insufficiency. The plaque appeared to be non-calcified and readily evident on ultrasound.
Measurements were performed with the penis fully erect (>90% erection): 20-degree dorsal curvature (pointing up) in the midshaft of the penis.
Given the recent onset of the problem and slight curvature, he was advised that the following treatment options for Peyronies disease are available to him: observation, penile traction therapy, injections with Xiaflex, verapamil, or interferon. Surgical treatment options including penile plication and plaque excision with grafting were discussed but given the recent onset of the problem surgical treatment is not advisable at this time. Xiaflex is typically used for more severe disease (curvature >30-degrees).
He was advised to start penile traction therapy for at least 6 months with the aim to prevent worsening of the problem and to improve existing curvature. He was advised tadalafil daily which has been shown to be helpful wit penile pain. Shockwave therapy for penile pain was also discussed as an option. Early initiation of penile injection therapy was given to the patient as an option.
Follow-up Appointment – 6 weeks
The patient started using a penile traction device. He decided to proceed with the early initiation of a series of 6 verapamil injections with the goal of improving penile curvature and preventing worsening of the problem.
Follow-up Appointment – 3 months
The patient completed treatment course with verapamil. He is still using penile traction therapy and Cialis daily. He reports improvement in penile pain with minimal pain with erections, pain score of 1-2 down from 5-6. He reports no significant change in curvature but no worsening of his symptoms. He was advised to continue tadalafil (Cialis) daily and penile traction. Physical exam revealed a slight decrease in plaque size.
Follow-up Appointment – 6 months
The patient is still doing penile traction and using tadalafil. The patient reports no pain in the penis. He feels that the curvature has improved.
Follow-up Appointment – 9 months
The patient reports no significant changes since the last visit. No new symptoms and no worsening of penile curvature. He has a slight curve which does not bother him and does not interfere with sex. The option of stopping tadalafil and penile traction was discussed with the patient. He was advised to come back and consider additional therapy if symptoms recur or worsen.
