Congenital Penile Curvature: What You Need to Know

Congenital penile curvature refers to the curvature or bend of the penis that is typically present from the time that the young man becomes sexually aware of the appearance of an erect penis, typically as a teenager or adolescent.  

Congenital penile curvature is different from the curvature caused by Peyronie’s disease. Curvature caused by Peyronie’s disease is typically an acquired curvature that occurs when there was no curvature previously.  Peyronie’s disease causes a new and often severe curvature caused by penile trauma or Peyronie’s plaque.

In other words, congenital penile curvature has been present from a young age.

It is important to recognize that most men have slight curvature or deviation of the penis when the penis is fully erect.  The penis may point to the right or the left when the penis is fully erect. This slight curvature does not cause problems and is not considered diagnostically the same as “congenital penile curvature,” which tends to be more severe and causes sexual difficulties.

In addition, it is important to understand that when the penis is not erect, the penis can point in any direction.  Penile curvature cannot be diagnosed based on the appearance of the penis when it is not erect.

The diagnosis of congenital penile curvature is based on the appearance of the penis in an erect state.  The severity of penile curvature can only be correctly judged when the penis is fully erect.  Chordee is a special type of congenital penile curvature that often occurs in conjunction with hypospadias.

Congenital penile curvature most commonly points down (ventral congenital penile curvature) but can also point up (dorsal curvature), to the sides, or sometimes can cause complex corkscrew-like curvature.

Causes of Congenital Penile Curvature

There are a number of different causes of congenital penile curvature which result in the same appearance.  For many men, there is no clear explanation of the reason for the penile curvature other than the presence of the curvature itself suggests that there is an asymmetry in the tension forces that occur in the erect state, which manifests as penile curvature.

In some men, the asymmetric distribution of forces from the penile skin to the shaft of the penis likely results from the asymmetric development of cremasteric fibers of the penis. Asymmetry in the connective tissue density of vector forces can cause congenital penile curvature.  In other men, there is either rotation or asymmetric development of the penile shaft, and in particular of the tunica albuginea of the penis.

Treatment of Congenital Penile Curvature

For men with congenital penile curvature who do not experience any sexual difficulties and are not bothered by aesthetic appearance, no treatment is necessary.  Reassurance that it is not a dangerous condition is often all that is needed.

Depending on the location of the curvature, most men with curvature greater than 20 to 40 degrees have significant bother from it.

For men who are bothered by the appearance of the penis caused by congenital penile curvature or if they experience difficulty with sexual activity or intimacy as a result of penile curvature, the curvature can be corrected.

Congenital penile curvature cannot be corrected by any creams. Penile traction devices can be used, but it is not clear whether penile traction devices are effective in the treatment of penile congenital penile curvature. Penile traction devices have been shown to be effective in the treatment of Peyronie’s disease curvature, but the nature of the disease process is different. No studies have been conducted in a randomized controlled fashion to specifically evaluate whether penile traction devices are effective for men with congenital penile curvature.

Congenital penile curvature is typically treated surgically. Often a stage-wise approach is taken with penile degloving as the first step, which sometimes leads to the resolution of penile curvature. Penile plication is often used for the correction of penile curvature with good results.

The short frenulum of the penis is a variation of congenital penile curvature.  Frenulum breve or short penile frenulum can be effectively treated with penile frenuloplasty, a 30-minute office procedure that can be performed under local anesthesia.

What to Expect Before and After Surgery for Congenital Penile Curvature

At New York Urology Specialists, surgical treatment for the correction of congenital penile curvature is usually performed in the office under local anesthesia. The procedure takes 1 to 2 hours to perform. For more complex curvature, the procedure is performed in a hospital under general anesthesia.

Whether the procedure is performed in the office or a hospital, the patient is usually able to go home the same day.

At New York Urology Specialists, we advise our patients not to go to the beach, swimming pool, or bike for 2 weeks. Men can take a shower the next day after the procedure. Most men can return to work within a few days after the procedure. Men are advised not to engage in sexual activity for 6 weeks after the procedure.

Most insurance plans cover the cost of surgical treatment for congenital penile curvature if it causes functional problems. Insurances do not cover the cost of the surgery if the reason for the surgery is cosmetic in nature.
