What is Urethral Meatal Stenosis?
Symptoms of urethral meatal stenosis in men include split urinary stream, slow stream, and need to push to urinate. A tiny opening of the urethra is the most obvious sign of urethral meatal stenosis. Phimosis can cause similar symptoms and can sometimes be mistaken for meatal stenosis.
What are the Causes of Urethral Meatal Stenosis
Meatal stenosis most commonly occurs in children after circumcision. In adults, meatal stenosis may be caused by balanitis xerotica obliterans (BXO). It may also be caused by prior surgical procedures such as urethral dilatation, cystoscopy, ureteroscopy, and endoscopic treatment of enlarged prostate with procedures such as TURP, Greenlight laser, Urolift, and Rezum.
Treatment Of Meatal Stenosis.
Urethral dilatation can offer immediate relief from meatal stenosis. In many situations, meatal stenosis will recur after urethral dilatation. Typically for severe meatal stenosis minimally invasive procedure called urethral meatotomy is performed.
Meatotomy can be performed in the office under local anesthesia in less than 1 hour. The success rate is greater than 85%, with less than a 10-15% chance of recurrence.
For patients with extensive and severe urethral meatus stenosis and stricture of granular urethra, more extensive urethral reconstruction is necessary. Urethroplasty with local flaps or using buccal mucosa is an effective treatment option. Depending on the extent of the problem, a two-stage procedure may need to be performed.
