Sexual Injuries in Men

Trauma during sex is not uncommon. Most injuries during sex are minor, such as bruising and bleeding, which can occur as a result of the tight foreskin (phimosis), balanitis (inflammation of the foreskin), or frenulum breve (tight frenulum of the penis). Penile fracture is less common but does happen occasionally.
Sexual injuries are not uncommon, but fortunately, most of them are minor. The most common sexual injury is a ‘friction burn” or laceration of the penile foreskin in men. Women can experience laceration of the vaginal mucosa. The problem is more common in uncircumcised men as the foreskin is more susceptible to lacerations.
The most reliable way to prevent friction burn is to make sure there is plenty of lubrication. When these problems are recurrent, they may be an indication of an underlying skin problem or an infection. In men, circumcision is often curative.
Laceration, Tears, and Bleeding Underneath the Penis: Tear of Frenulum Breve
Other common injuries in men include a painful tear of the frenulum breve (short frenulum of the penis) on the underside of the penis, which can be cured with penile frenuloplasty (a 20-minute in-office procedure). We see many men who have suffered from this problem for a long time, not realizing that a simple and effective cure is available.

Pain in the Penis During Sex
There are many causes of pain in the penis during sex. These can include skin problems such as lacerations, infection (balanitis), scar formation in the penis called Peyronie’s disease.
Penile fracture is an emergency and requires urgent treatment. If untreated, it may often lead to erectile dysfunction (difficulty getting/keeping erections). There is also a risk of developing scar tissue in the penis, a lump that is called “Peyronie’s Plaque,” which can lead to a bent penis, shortening and narrowing of the penis, and difficulty with erections – Peyronie’s disease.
Typically, a penile fracture occurs when the penis slips during thrusting and hits the partner’s perineum, causing high-pressure injury to the penis. The man typically experiences sharp pain; sometimes, blood in the urine may occur, and black-and-blue can occur. Penile fracture is an emergency situation, and a prompt visit to the ER or a urologist is advised. There is a risk of scarring developing that can lead to erectile dysfunction (difficulty with obtaining and maintaining erections) and Peyronie’s disease (penile curvature, pain, or narrowing). If a penile fracture occurs, surgical treatment may often be necessary to minimize the risk of complications.
