Priapism Treatment: 49-year-old Man Treated at New York Urology Specialists

JR is a 49-year-old man with multiple episodes of prolonged erections over the past few years after using penile injections (Trimix).  He had an episode of an erection lasting more than 24 hours approximately 2 years ago.  He was treated at a hospital with a penile shunt procedure.  He reports that he has been unable to obtain erections subsequently.  He presented to New York Urology Specialists for a second opinion.  He complains of penile shortening.

Physical exam revealed fibrotic, scarred penile shaft.

Penile duplex ultrasound after Trimix ejection revealed scar tissue in the penis; poor response to Trimix administration.

Treatment options were discussed with the patient. Because of the severe fibrosis and poor blood supply, medications such as Viagra and tadalafil (Cialis) do not work in his situation. Shockwave therapy is not an option because of a lack of circulation and extensive scar formation. The patient had a poor response to Trimix injection, which excludes it from being a viable option.

The patient was explained that the only feasible option is to implant a penile prosthesis. Because of extensive fibrosis and scar formation, the penile implant procedure is more challenging to perform. The ideal time to place a prosthesis is shortly after the development of prolonged priapism lasting more than 12 to 24 hours with subsequent development of erectile dysfunction before scarring and fibrosis had a chance to affect the penile shaft.

The patient decided to proceed with penile implant surgery.  He was advised to use a penile traction device in an effort to lengthen the penile shaft prior to surgery.  The length of the penile implant is limited by the functional length of the penis.

Follow-up Appointment – 2 months

The patient underwent insertion of inflatable penile prosthesis AMS 700CX which provides excellent rigidity to contract effective scar tissue. Extensive scarring was noted during the procedure.

Follow-up Appointment – 3 months

The patient came back 1 month after penile implant surgery. He reported some pain during the first week after surgery. At this point, he reported mild discomfort in the scrotum but much less discomfort than previously. The incision has mostly healed. He was advised that he can resume sexual activity in 2 weeks.  He was taught how to inflate and deflate the penile implant again.

Follow-up Appointment – 6 months

The patient returns more than 3 months after penile implant surgery. He is very satisfied with the quality of erections. He is able to inflate and deflate the implant without difficulty.

His only concern is that his penis is not as long as it was prior to priapism.  He has lost over 2 inches of length compared to what his penis was 3 years ago.

The patient was advised that while the penile implant can provide rigidity that is needed for penetration, a penile implant cannot make the penis longer than what it is or return it to the length prior to the shortening of the penis that occurred as a result of penile scarring after priapism.
