Can You Cure Mycoplasma?
M. Genitalium Treatment
There are several classes of antibiotics that are effective against Mycoplasma:
- Tetracyclines (doxycycline)
- Macrolides (azithromycin)
- Fluoroquinolones (levofloxacin, moxifloxacin)
Doxycycline for Mycoplasma has a cure rate of 30 to 40 percent. Studies show that azithromycin has a better cure rate than doxycycline (link) and clears the infection in 85 to 95 percent of people (link). Therefore, at New York Urology Specialists, azithromycin is the first line of treatment for Mycoplasma.
Azithromycin is prescribed either as a single-dose regimen with a 1-gram dose or as an extended-dose regimen taken over 5 days. The single-dose regimen of azithromycin has cure rates of 60 to 85 percent (link). Cure rates have been declining due to macrolide resistance when Mycoplasma is treated with the single-dose azithromycin regimen without performing a test of cure. The extended-dose regimen of azithromycin has higher cure rates and is therefore preferred.
At New York Urology Specialists, we prefer a modified extended treatment with Azithromycin as the first-line treatment for M. genitalium for women and men. More complex problems require an individualized approach.

Treatment of Persistent Mycoplasma Genitalium Infection
Fluoroquinolones are the second line of treatment for Mycoplasma. Moxifloxacin 400 mg once a day for 7-10 days orally has a cure rate of as high as 95 percent (link). Because of potential side effects, moxifloxacin is typically prescribed for patients who fail treatment with other antibiotics.
If treatment with doxycycline, azithromycin, and moxifloxacin fail, the third line of treatment for persistent Mycoplasma genitalium may include newer antibiotics such as pristinamycin 1 gram four times daily orally for 10 days with a cure rate of 90 percent (link). However, pristinamycin is only available in France; other options are available in the US.
Complicated Mycoplasma infections (pelvic inflammatory disease in women or epididymitis in men) are treated with an extended course of moxifloxacin 400 mg orally once a day.
