What You Need to Know about Treatment of  Phimosis in Men

What is Phimosis?

Phimosis refers to the narrowing of the foreskin at the tip of the penis that prevents the skin from being pulled back. It affects 8-10 percent of men over their lifetime. The un-retracted prepuce (foreskin) may appear like a tight rubber band or ring around the tip of the penis.

In babies and young children, it is normal for the foreskin to be un-retractable.  This is called physiological phimosis. Normally, as boys grow older, the foreskin loosens and can be pulled back for a cleaning.

What Are the Causes of Phimosis?

Congenital phimosis is the most common cause among adult men. Typically, men are born with the condition and have never been able to pull the skin back for cleaning or during sex. Often, when it is lifelong, the condition is severe, and sometimes only the urethral opening is visible.

As men grow older, 5-10 percent of uncircumcised men who previously had no trouble pulling the foreskin back develop inflammation and scarring of the foreskin that leads to phimosis and an inability to pull the foreskin back. When phimosis is severe, the foreskin cannot be pulled back easily with the penis flaccid (not erect). When phimosis is not as severe, the foreskin may be pulled back when the penis is not erect, but the foreskin cannot be pulled back when the penis is erect and during sex.

The most common causes of acquired phimosis are diabetes, obesity, and penile trauma or frictionBalanitis xerotica obliterans (BXO), poor hygiene, and penile cancer can also lead to this condition. Foreskin infection with bacterial and fungal organisms (balanitis and balanoposthitis) often lead to inflammation and scarring of the foreskin, which ends up preventing the foreskin from being retracted.

Symptoms of Phimosis

The most common symptom of phimosis is an inability to pull back the foreskin. Many men with this condition have a visible and palpable phimotic ring, which is a circumferential ring of foreskin encircling the penile head. The phimotic ring is typically thickened and feels harder due to the scarring of the area.

There may be swelling and redness in the area of the phimotic ring. Transient ballooning of the foreskin may occur during urination, causing difficulty with urination or infection and warranting treatment. It is important to remember that forceful retraction of the foreskin should not be attempted as this can be painful and may lead to bleeding and scarring.

Recurrent Phimosis in Previously Circumcised Men

A small number of men who have previously been circumcised may develop phimosis. This typically occurs in men 40-60 years old who gain a significant amount of weight or develop diabetes. It can also occur in younger or older men.  The extra skin that is pushed forward by excessive fat covers the glans of the penis and can scar, causing recurrence of the condition.

Treatment of Phimosis in Children

Is it necessary to treat phimosis?

In babies and very young children, treatment is not necessary as the condition typically resolves spontaneously. In babies and children with recurrent urinary tract infections, circumcision may be curative and prevent recurrent infections.

In older children, corticosteroid creams, such as 0.05% betamethasone or 1% hydrocortisone cream, which are available over-the-counter, in combination with daily gentle manual stretching of the foreskin by the parent, will lead to resolution of phimosis is about 30-50 percent of boys. If steroid cream does not help, then circumcision is a very effective treatment option in children.

Treatment of Phimosis in Adults

In adult men, the condition tends to be associated with a more pronounced phimotic ring and more pronounced scar tissue in the foreskin. As a result, anti-inflammatory corticosteroid creams are not as effective in adult men, in particular in men with severe phimosis and a pronounced phimotic ring. In adult men, often, there is associated balanitis (infection and inflammation of the foreskin).

Can you treat phimosis at home? In young children without symptoms (infection, pain), we can take a wait-and-see approach as the condition may resolve on its own. In older children and adults, the condition does not usually improve without treatment.

Medications

Anti-inflammatory medications (topical creams) that are antibacterial/antifungal are effective if an infection is present. A 1% cortisone cream or 0.05% betamethasone cream in combination with an antifungal cream (clotrimazole) is an effective option for men with newly developed phimosis typically caused by an infection. For men with recurrent inflammation and infection of the foreskin or recurrent phimosis, creams are typically not very effective as the problem will recur after the creams are stopped.

Penile Stretching Devices

Several penile stretching kits are being sold online. These kits may be helpful to some men with very mild phimosis, but typically the benefit does not last, and there is a high frequency of recurrence. Eventually, most men who try penile stretching do not derive sufficient benefit from it and require circumcision.

Circumcision

The definitive treatment for phimosis is circumcision, which is a minor surgical procedure in which the foreskin is partially or completely removed. This is typically the most effective treatment option. The success rate of circumcision in curing phimosis is greater than 98 percent.

Phimosis Present Only with Erect Penis

Some men have a milder form of phimosis, which prevents the foreskin from being pulled back when the penis is erect during sexual intercourse. Often, there is a noticeable phimotic ring that prevents the foreskin from being pulled back.

Some men have a short penile frenulum (frenulum breve), which can cause pain with erections and during sex and can prevent the foreskin from being pulled back. Fairly frequently, phimosis and frenulum breve may coexist and may require circumcision with frenuloplasty to address both problems, which can be performed simultaneously in one setting.
