What You Need to Know about Preputioplasty
What is Preputioplasty?
Preputioplasty is a procedure that can be performed for men with mild phimosis (narrowing of foreskin that prevents foreskin from freely pulling back). Preputioplasty involves making a small incision in the narrowed or scarred foreskin and then reconstructing it in a way that minimizes the risk of recurrent scarring.
When Is Preputioplasty Performed and What Does It Treat?
Preputioplasty is a treatment option for mild or minimal phimosis for men. It’s an option for men who have phimosis but who do not experience frequent inflammation and irritation of foreskin caused by infection (men who do not have fungal balanitis). Preputioplasty is an alternative to circumcision for men who do not want to have a circumcision.
How Successful is Preputioplasty For Treatment of Phimosis?
Preputioplasty success in treating phimosis depends on the severity of underlying phimosis. For patients with mild phimosis who have mild discomfort when they pull the foreskin back, preputioplasty of the penile foreskin (prepuce) can be successful 70-80% of the time.
Patients with extensive scarring have low success rates with preputioplasty. Men with fibrotic and scarred foreskin are advised to have a circumcision. In general, the success rate of circumcision for treatment of adult male phimosis approaches close to 100%, a much more successful treatment option than preputial plasty.
Men with the fibrotic (hard, scarred) phimotic ring are not good candidates for preputioplasty as the problem is very likely to recur and would require circumcision to correct it.
Men with Balanitis are Not Good Candidates for Preputioplasty.
In general, the most common type of balanitis in men, fungal balanitis, is caused by the moist environment within the foreskin. Preputial-plasty does not treat the underlying problem and will not prevent the recurrence of balanitis. Balanitis is especially prevalent in diabetic men.
Preputioplasty, in general, is not advised in men with balanitis as these problems (both balanitis and resultant phimosis) are likely to recur. For men with balanitis, sleeves circumcision or ShangRing circumcision offer effective and nearly painless treatment options with fast recovery.
Alternatives to Preputioplasty and Full Circumcision
Some men have a problem that is too severe to be effectively treated with preputioplasty, and they want to maintain the foreskin. It is possible to perform a partial circumcision or ‘loose circumcision‘ by removing the offending skin that causes phimosis and maintaining as much of the normal skin as possible.
