What You Need to Know About Penile Frenulum
The frenulum of the penis is a skin tag that is normally present on the underside of the penis. The penile frenulum connects the glans penis to the foreskin.
In circumcised men, the frenulum may or may not be present depending on whether it was removed during circumcision. Sometimes the frenulum is partially or fully removed during both neonatal and adult circumcision. Other times, either the entire frenulum or part of the frenulum is left behind after circumcision.
In some men, the skin tag is minimally noticeable; in other men, the penile frenulum is very prominent. You can view examples of the frenulum appearance in our image gallery.

What is Frenulum Breve or Short Frenulum of the Penis?
Normally, the frenulum does not cause any problems. In some men, the frenulum is too short from birth (frenulum breve or short frenulum). In other men, the frenulum may shorten as a result of repeated trauma (stretching) from sex.
A short and tight frenulum can cause pain in the penis with erections and during sex. This can occur even in previously circumcised men who did not have a complete removal of the frenulum during circumcision. In 2-5% of men who undergo treatment of frenulum breve with frenuloplasty or frenulectomy, the condition may persist, and retreatment may be necessary.
Some uncircumcised men who have phimosis (cannot pull back the foreskin) are not aware that they have frenulum breve because they have never been able to pull back the foreskin. In men with severe phimosis, the tight frenulum is discovered only when they undergo circumcision for the treatment of phimosis.
Cuts, Bleeding, and Lacerations of the Penile Frenulum
The only effective treatment for frenulum breve (short frenulum) is a minimally invasive surgical procedure that is known as frenuloplasty or frenulectomy (sometimes spelled as a frenectomy or frenulotomy). Frenuloplasty can be performed alone or in combination with a circumcision procedure. For men who have co-existing recurrent balanitis, BXO, or long-standing tight phimosis, circumcision should be performed at the same time as frenuloplasty.
Men who have no evidence of phimosis can choose to have frenuloplasty. If a man desires to have a circumcision for hygienic or cosmetic/aesthetic reasons, it can be performed at the same time as frenuloplasty.
Men who have a tear of the frenulum sometimes are cured as a result. If a tear of the frenulum repeatedly recurs after it heals, frenuloplasty can be performed to cure the problem. Repeated tearing and bleeding cause more scar formation and lead to further shortening of the frenulum; it is unlikely that it will heal on its own if repeated tears occur. Alternatively, frenuloplasty can be performed within days of a frenular tear to cure the problem.
What are the Symptoms of Frenulum Breve or Short and Tight Frenulum of the Penis?
The tight and short frenulum of the penis often causes tightness with erections when the foreskin is pulled back for urination or during sex. Often, men do not discover that they have a problem until they become sexually active. Sometimes frenulum breve symptoms are noted only when sex without condoms occurs.
