Culture versus DNA Testing for Ureaplasma: How to Test for Ureaplasma?

Ureaplasma testing can be performed with a urethral swab or urine specimen. Traditionally, urethral swabs for Ureaplasma were done in men and vaginal swabs in women. 

Modern DNA testing made urethral swabs and vaginal swabs largely unnecessary. Studies show that the success rate of detection is the same in a urine sample, so it’s no longer necessary to do swabbing. In most people, Ureaplasma can be detected from a urine sample.

There are two ways of testing for Ureaplasma infection, culture or DNA/RNA testing. At New York Urology Specialists, we do DNA/RNA Ureaplasma testing because it is a more reliable method with a detection rate of up to 98 percent.  Mycoplasma infection often co-exists with ureaplasma and we routinely test for mycoplasma infection as well.

The PCR test looks for DNA of the bacteria in the urine sample of the patient. PCR assays are more sensitive and specific compared to culture (link). DNA testing can detect Ureaplasma infection even in the presence of other infectious agents. PCR assay is much quicker than culture. DNA testing for Ureaplasma in men is recommended because sometimes the infection remains undetected on culture.

Why Was I Not Tested for Ureaplasma?

The majority of individuals who carry Ureaplasma in their body do not have any symptoms. Symptoms only develop when the person has a high bacterial load. Extensive testing and treatment of Ureaplasma is an unnecessary cost and can lead to antibiotic resistance in these bacteria and other sexually transmitted infections (link). Routine screening for Ureaplasma in men and women with no symptoms is not recommended.

If symptoms are present, testing for Ureaplasma is usually recommended in conjunction with testing for other STDs like gonorrhea and chlamydia. Treatment is offered if a person is found to have a high bacterial load of Ureaplasma.
