How the AMS-800 Works

The AMS-800 Artificial Urinary Sphincter is a surgically implanted medical device designed to mimic the function of a healthy urinary sphincter. It consists of three components: a cuff that wraps around the urethra, a pump placed in the scrotum or labia, and a balloon reservoir located in the abdomen. The cuff remains closed to prevent urine leakage during normal activities. When the patient needs to urinate, they manually squeeze the pump, which temporarily opens the cuff, allowing urine to flow. After urination, the cuff automatically recloses, restoring continence. This mechanism provides reliable control for patients with severe SUI.
 

Who is a Candidate for AMS-800 Artificial Urinary Sphincter (AUS)?

The AMS-800 Artificial Urinary Sphincter (AUS) is an option for many patients with severe stress urinary incontinence. Ideal candidates include:

  • Men with SUI (stress urinary incontinence) following radical prostatectomy for prostate cancer 
  • Patients who have undergone radiation treatment after radical prostatectomy have developed stress urinary incontinence.
  • Individuals with SUI after transurethral resection of the prostate (TURP), HOLEP, laser enucleation of the prostate, Greenlight laser, or other pelvic surgeries.
  • Some women with severe SUI, particularly when the urethral sling failed.

Risks and Complications of AUS

While the AMS-800 Artificial Urinary Sphincter (AUS) is highly effective for many patients, it carries potential risks and complications, including:

  • Infection: Surgical implantation can lead to infections, requiring antibiotics or, in rare cases, device removal. The infection risk is typically low, and we administer antibiotics to further minimize the risk. 
  • Device malfunction: Mechanical issues, such as pump or cuff failure, may necessitate revision surgery.
  • Erosion or tissue damage: The cuff may erode into the urethra or surrounding tissues, causing discomfort or requiring surgical correction.
  • Tissue atrophy: The urethral tissue may atrophy over time, making the AUS less effective. Surgical revision or replacement may be necessary.
  • Urinary retention or obstruction: urinary retention is rare and usually temporary. 
  • Need for revision surgery: Over time, the device may require adjustments or replacement due to wear or complications. Depending on the particular situation, 30-60% of men require revision or replacement of the implant over time. The risk is higher in patients who have had prior radiation.
