The Altaviva Tibial Implantable Neuromodulation: What You Need to Know

New! Dr. Alex Shteynshlyuger now offers the new tibial implantable neuromodulation, Altaviva device, for the treatment of urge urinary incontinence. We continue to offer the Interstim sacral modulation for men and women.
Benefits of the Altaviva Tibial Implantable Neuromodulation
- Minimally Invasive Procedure with Quick Recovery. The procedure can be performed in our office in 15 minutes under local anesthesia. The outpatient procedure does not require general anesthesia, sedation, or imaging guidance.
- Patients typically go home the same day with therapy already activated—no waiting period or trial phase like some other neuromodulation options.
- The device itself is compact (roughly the size of a small USB flash drive), making it discreet and low-profile under the skin.
- MRI-compatible device
- Recharge needed once a year
Effectiveness of Altaviva for Treatment of Urinary Urge Incontinence
In the Titan 2 study, published in the Journal of Urology in 2026:
- 80% of patients reported improvement in symptoms, with 60% experiencing greater than 50% reduction in urge urinary incontinence episodes
- 20% completely dry (no urinary incontinence episodes).
- 51% report “much better” or “very much better” at 1 year
Risks and Side Effects of Altaviva
There is about 7% risk of wound infection, 1% risk of implant site discomfort, and a 1-2% implant site pain
Does Insurance Cover AltaViva Tibial Nerve Neuromodulation for Urinary Problems?
Altaviva tibial nerve neuromodulation is an effective treatment option that has been shown to be effective in high-quality randomized controlled trials. Altaviva is FDA-approved for the treatment of urinary urge incontinence in men and women. Health plans are expected to cover treatment with Altaviva tibial neuromodulation as long as plan-specific medical-necessity criteria are met. New York Urology Specialists will work with your health plan to obtain Altaviva coverage, and if necessary, will file an appeal if the treatment is denied.
Typically, insurance coverage requires a comprehensive evaluation (physical exam, labs, pelvic ultrasound, cystoscopy, and possibly urodynamic testing). In addition, adequate trial of behavioral therapy and oral medications is required by many health plans, including Medicare, Aetna, Cigna, United Healthcare/Oxford, and Blue Cross plans, including Anthem BCBS.
