TESA (Testicular Sperm Aspiration): What You Need to Know

TESA (Testicular Sperm Aspiration) is a procedure that is performed for men with infertility caused by obstructive azoospermia.
TESA is appropriate in situations when there are a sufficient number of sperm in the testis, but they cannot get out due to obstruction.
The most common situation when TESA is performed is for men with a prior vasectomy who need sperm for IVF (In-vitro fertilization).
Sperm obtained via TESA can ONLY be used for IVF. It cannot be used for IUI (intra-uterine insemination).
TESA (Testicular Sperm Aspiration) should not be confused with similarly named procedures that are performed for different reasons, such as TESE (Testicular Sperm Extraction) or MicroTESE (microscopic TESE). TESE is usually performed for men with non-obstructive azoospermia (few sperm inside the testis).
Testicular fine-needle aspiration (TEFNA) is a procedure similar to TESA, except it uses a smaller needle and tends to retrieve fewer sperm.
TESA Procedure at New York Urology Specialists
- Typically, TESA is performed in our office under local anesthesia. It is possible to provide IV sedation anesthesia, but usually, it is unnecessary.
- Men’s health needs to be optimized to ensure optimal retrieval of sperm. Men with prior testosterone replacement (TRT) need to stop testosterone and transition to “sperm-safe” methods of raising testosterone levels (HCG, clomiphene, enclomiphene, etc.)
- There is no preparation necessary on the day of the procedure if it’s performed under local anesthesia.
- Local anesthetic is administered in the inguinal canal to provide a ‘spermatic cord block” which causes testicular anesthesia.
- An additional anesthetic is administered to the scrotal skin.
- The sperm is aspirated (retrieved) from the testicle with a needle.
- A dressing is applied to minimize bleeding.
Once the sperm is retrieved, it is placed in ‘sperm washing media.” We typically use Irvine Scientific sperm wash media, but some IVF centers prefer other media. The patient or his partner takes the specimen to their IVF center or semen storage facility (cryopreservation). This sperm can be used fresh for IVF or frozen for future use in IVF.

Typically, 90% of the type adequate sperm can be retrieved via TESA from patients who are appropriate candidates for this procedure.
