Circumcision Styles:
Circumcision is classified by the location of the incision (scar) into high circumcision and low circumcision.
Low circumcision refers to a scar line that is close to the glans of the penis.
High circumcision refers to a scar line that is closer to the base of the penis.
It is important to understand that most of the time the difference between low circumcision and high circumcision is about 1 to 2 cm or about 1/2 of an inch.
There is another option to perform circumcision at the base of the penis, where the penis meets the scrotum, however, that has limitations as it cannot treat conditions such as recurrent balanitis (inflammation, phimosis, and frenulum breve).
In general, for patients undergoing circumcision for cosmetic reasons the incision can be made anywhere, within reason. On the other hand, the location of the incision and whether the circumcision is low or high is limited by the need to cure the underlying problem in men with balanitis, phimosis, BXO of the penis, and tight frenulum of the penis.
Circumcision can also be classified by how much foreskin is removed into “tight circumcision” and ‘loose circumcision‘.
Circumcision style does not affect the function of the penis. It does not affect the quality of erections or sensitivity of sensation. Circumcision style mostly has to do with preferences about the appearance and location of the circumcision scar.

Circumcision Techniques
Circumcision can be performed using multiple techniques: clamp circumcision and sleeve circumcision are among the most common techniques used for circumcision.
Clamp Circumcision
Circumcision can be performed using clamps. For example, a GOMCO clamp is commonly used for neonatal (baby) circumcision. In adults, clamps are less commonly used in the United States or Europe but have been used in less affluent countries due to low cost. Typically, a clamp is placed on the penis and the skin eventually falls off as a result of strangulation of the blood supply to the foreskin.
A more innovative clamp-type circumcision procedure called ShangRing circumcision is an updated variation of clamp circumcision. ShangRing circumcision is offered at New York Urology Specialists.
Dorsal Slit
A dorsal slit is a technique often used as part of circumcision in particular in men with phimosis. Typically a sleeve circumcision is performed after dorsal slit. Dorsal slit without circumcision can be performed but results in a less pleasing appearance.
Sleeve Circumcision
Sleeve circumcision is the most common type of circumcision performed in adults and older children by urologists. Sleeve circumcision involves making two circumferential marks on the penis and removing intervening foreskin between these marks. The edges of the incisions are sutured together. Sleeve circumcision can be used for cosmetic circumcision, treatment of phimosis, balanitis, and BXO. Sleeve circumcision is a very versatile procedure and can be combined with other procedures to achieve the desired result.
