Treatment of Hyperactive Cremaster Muscle: A 42-year-old man treated at New York Urology Specialists.
JI is a 42-year-old man with complaints of groin and testicular discomfort that occur when the testis is pulled up into the groin. The problem occurs often and has been present for >10 years. He reports that he loses erections occasionally due to this discomfort. Occasionally takes Tylenol, which helps a bit with the discomfort.

The patient was offered the following treatment options for hyper-reflexic cremaster muscle available to him: a trial of anti-inflammatory medications / NSAIDS, muscle relaxants, and surgical treatment with cremaster muscle fiber lysis/release.
Follow-up Appointment – 1 month
At the follow-up visit 1 month later, RF reports that he tried oral medications but did not experience a lasting benefit. Surgical treatment was discussed with the patient, in particular, microsurgical subinguinal cremaster muscle release (MSCMR). The patient elected to proceed with the surgery.
He was advised to obtain a medical clearance from the primary care doctor, including an EKG. Prior authorization was requested from his insurance, which was approved 3 weeks later after an appeal.
Surgery Appointment
The patient underwent microsurgical subinguinal cremaster muscle release (MSCMR) surgery under general anesthesia. He was discharged home a few hours after the procedure.
Follow-up Appointment – 2 weeks
The patient returned 2 weeks after the procedure. He had some pain for the first few days after the procedure and took pain medications, but has not taken any pain medications for the past week. He was able to return to work on Monday (the procedure was done on a Friday).
Follow-up Appointment – 2 months
The patient returned 2 months after the MSCMR procedure. He reports >90% resolution of the symptoms. No longer takes medications.
