Treatment for ED: What You Need to Know

Penile prosthesis surgery allows men with erectile dysfunction to obtain rigid erections. The penile implant helps men who are unable to obtain erections sufficient for intercourse to obtain strong erections sufficient for penetration.

A penile implant is an effective treatment option for obese men suffering from erectile dysfunction.

Preparation for Penile Implant

Ideally, men should try to lose weight prior to surgery. Men with severe, morbid obesity may benefit from bariatric surgery to help them with weight loss if they are unable to lose weight on their own. The benefits of bariatric surgery include weight loss, prevention, and reversal of diabetes and metabolic syndrome, and improvement in control of high blood pressure.

Men with excessive skin around the penis after weight loss may benefit from the removal of the excessive skin, which can help with maximizing the length of the penis.

Penile Implant Choices in Overweight and Obese Men

Overweight men with erectile dysfunction are typically advised to lose weight. Men with low testosterone may benefit from testosterone replacement therapy, which can also help with weight loss.

The choice of a penile implant is usually based on patient preferences. Most men with good manual dexterity who can easily reach the scrotum to inflate and deflate the pump of the inflatable penile implant are offered either the AMS-700 penile implant or the Coloplast Titan penile implant.

Men with severe obesity who are unable to lose weight or unwilling to undergo bariatric surgery prior to penile implant surgery often have difficulty reaching the scrotum due to obesity. As a result, they may not be able to inflate and deflate the inflatable penile prosthesis which does require some manipulation of the scrotum. For this reason, in men with severe obesity, we typically recommend a malleable, non-inflatable penile prosthesis which is simpler to use, especially for men who have difficulty reaching and manipulating their scrotum.
