Occasional Tears in the Area of Penile Frenulum
ME is a 36-year-old man. He has been experiencing occasional tears in the area of the frenulum of the penis, which he researched and self-diagnosed as ‘frenulum breve‘. He presented with a request for penile frenuloplasty and circumcision. His girlfriend does not like the foreskin as it interferes with sexual activity. He has not had any previous treatment.
Physical examination revealed a frenulum breve of the penis and normal foreskin. After a discussion of treatment options, the patient decided to proceed with penile frenuloplasty and circumcision.
He underwent adult cosmetic circumcision and penile frenuloplasty of the penis. The procedure was done under local anesthesia and took just over one hour. The patient was able to go home the same day. He described pain score during the procedure as 2 out of 10 (1= no pain; 10 = worst pain of his life). He did not require any pain medications at home. The patient has an office job and was able to return to work in 1 day.
On follow-up 2 weeks after the procedure, he had no complaints other than difficulty with urination from the dressing. He had mild discomfort in the first 2-3 days and took a few pills of Motrin.
The patient was seen 4 weeks later. The scar was well healed. A few remaining sutures were removed. The patient was advised that he can resume sexual activity in 1 week.
On follow up 3 months later the patient reported no complaints, no pain, no sensitivity. He was satisfied with the outcome of circumcision. He has not experienced any tears during sex. The patient was advised that no further follow up is necessary as everything has healed well.
