Testicular Prosthesis Insertion: Surgical Considerations
The most common complications of testicular implant surgery are infection and erosion or extrusion of the prosthetic testicle through scrotal skin. Scar formation, especially in a high-riding position, can also cause discomfort or a less than desirable appearance.
Infection after Testicular Implant Surgery
The biggest risk with testicular implant placement is an infection. There are two types of infections that can occur. The first is a superficial wound infection that occurs in 1-2% of patients. Typically, if the infection is isolated to the wound and does not involve deeper tissues or the prosthesis itself, it can be treated with antibiotics and successfully cured without the need to remove the prosthesis.
If the infection involves the testicular implant, antibiotics are ineffective, and removal of the prosthesis is always necessary.
If the surgical incision is located at a distance from the testicular implant and separated from the implant by layers of tissue, should a wound infection occur, treatment with antibiotics can often save the day by effectively treating the superficial wound infection. As long as the infection does not involve the prosthesis, antibiotics are very likely to cure the wound infection, and the patient will recover well.
When the surgical incision is made on the scrotum, the incision sits right on top of the implant. Most surgical site infections or incisional infections in patients where the incision is on the scrotum (adjacent to the implant) will result in simultaneous infection of the testicular prosthesis, which will necessitate the removal of the implant in order to treat the infection.
Extrusion and Erosion
In children undergoing testicular implant placement, a scrotal incision is associated with a significant risk of extrusion or erosion of the prosthesis due to thin pre-pubertal skin. Studies have found that almost all extrusions in children occur when the implant is placed through a scrotal incision. Similarly, the risk of extrusion is higher in a previously operated scrotum. In patients who have undergone radiotherapy for testicular cancer (not something that is typically done in the US), the scrotal skin is compromised, and there is an increased risk of extrusion if the implant is placed through a scrotal incision.
Testicular prosthesis replacement is usually performed through a sub-inguinal incision.
Treatment of Testicular Implant Infection
If a testicular implant gets infected, removal of the prosthesis is always necessary. In certain circumstances, if the infection involves only the wound but does not go deeper and does not involve the prosthesis itself, treatment of the wound infection is possible and may avert the need to remove the implant. Typically, this is only possible when the incision is made in the groin or sub-inguinally, at a distance from the implant. Most of the time when the incision is made on the scrotum, and a wound infection occurs, the testicular implant gets infected because it is abutting the surgical site.
If a scrotal incision was made to place the implant initially, removing the infected prosthesis through the same incision is advisable, along with thorough irrigation of the wound. Typically, we recommend waiting about 6-8 weeks for the infection to resolve and returning to the operating room in 6-8 weeks for the replacement of the testicular implant. It is important not to use the same scrotal incision for prosthesis replacement.
In particular, if the implant was placed initially through a scrotal incision, we would always use a subinguinal incision for testicular implant replacement. However, if a subinguinal incision was used initially, it is reasonable to reuse the same incision for the replacement of the testicular implant.
In certain situations, when the infection is not severe, it may be possible to replace the prosthesis after thorough irrigation of the surgical field, with a reasonably good success rate.
