Penile Curvature in Men
Bent or curved penis is a common symptom of Peyronie’s disease. Curvature can be up (towards the head) which is called “dorsal penile curvature“. When the curvature points the penis down, the curvature is called “ventral curvature”
Treatment options are similar but somewhat different for men with ventral and dorsal curvature.
In general, more treatment options are available to men with a dorsal curvature of the penis, which is also a more common type of penile curvature accounting for 80% of men with penile curvature.
The severity of dorsal curvature varies from mild to moderate to severe. Location of curvature also matters with curvature typically causing more difficulty with sex when it’s located distally on the penis or towards the tip of the penis.
Some men experience penile pain in addition to having a bent penis. Penile pain is another common sign of Peyronie’s disease.
Evaluation of Men with Dorsal Penile Curvature

If a lateral curvature with a deviation of the penis to the right or left side co-exists with dorsal curvature, a more complex measurement is performed.

Treatment Options for Dorsal Penile Curvature
The dorsal curvature of the penis (pointing up or to the head) is usually caused by a plaque on the dorsal side of the penis. Many men are able to feel a ‘knot’ of Peyronie’s plaque but some men do not have a palpable plaque.
Depending on the severity and duration of symptoms, treatment options for dorsal penile curvature include:
- Oral therapy
- Intra-lesional plaque injections (injection of medications directly into the scar tissue that causes curvature)
- Penile plication – a surgical procedure for the treatment of bent penis designed to straighten the penis.
- Plaque incision/excision with grafting – a surgical procedure that removes the scar tissue that causes penile curvature and narrowing.
- Penile implant
More severe and complex curvature is often associated with significant penile shortening. A combination of penile traction and surgical treatment is often used to treat severe cases.
Dorsal Penile Plaque
Dorsal plaque usually causes the penis to point up or to the side. Sometimes the curvature may be multi-dimensional pointing both up and to the side. Occasionally the curvature maybe even more complex ‘doughnut-like” causing severe multi-dimensional curvature.
Distal Dorsal Peyronie’s Plaque
Location of curvature whether closer to the body or closer to the tip of the penis affects the severity of sexual problems experienced by men. For the same degree of curvature, curvature that is closer to the tip of the penis tends to affect men more severely in terms of sexual function than the same curvature that occurs closer to the base (or beginning of the penis).
