Enlarged ProstateGreenlight PVP laser and Holmium laser are used for the treatment of symptoms of enlarged prostate and BPH in men.  Greenlight laser, in general, is a very safe and effective treatment option for BPH, as is Holmium laser.

Side Effects and Complications from Treatment for Enlarged Prostate with Greenlight Laser Surgery

Some men develop side effects and complications after treatment for BPH with Greenlight laser or other lasers used for the treatment of enlarged prostate.

These complications are not unique to the Greenlight laser procedure and can occur after any transurethral procedure performed on the prostate (Greenlight laser, holmium laser, any other laser procedure, or TURP).

There several side effects and complications that could occur after laser treatment of a large prostate.  The most common side effects and complications include persistent hematuria or bleeding, persistent frequent urination and urgency to urinate, urinary incontinence or leakage, and burning with urination.

Urethral Stricture

Slow urine stream or inability to urinate after Greenlight laser may be an indication of urethral stricture.  Often men are able to urinate for a few weeks or months after surgery and then their symptoms get worse.  The urine stream may split or deviate to the side; the urine stream may also become narrower or weaker.  This requires prompt evaluation and treatment. Cystoscopy is usually performed to help with diagnosis.

Urinary Retention after Greenlight PVP Surgery

Infrequently, men might have persistent urinary retention after Greenlight PVP surgery.  In this situation, re-evaluation is necessary.  Some men have poorly functioning bladder muscles.  Treatment for non-obstructive urinary retention using Interstim sacral neuromodulation may be useful.  Some men have persistent obstruction.  Re-operation may be necessary if obstruction persists. Infrequently, bladder neck contracture or urethral stricture may block urine flow, and these require prompt treatment.

Frequent Urination and Urgency After Greenlight Laser

Frequent urination and urinary urgency after Greenlight laser often occur immediately after surgery.  These symptoms should dissipate fairly soon, typically in the first 4-8 weeks.  The persistent or worsening urinary frequency may indicate incomplete bladder emptying or underlying partial urinary retention.  Persistent or worsening urinary symptoms may also indicate the presence of urethral stricture.  In men who had a large median lobe these symptoms may be a result of irritability of the bladder trigone that can occur after surgery.

Urinary Incontinence after Green Light Laser

Some men may develop urinary incontinence after treatment with Greenlight laser for enlarged prostate.  Most often this is caused by persistently high voiding pressure caused by overactive bladder and is treatable with medications used for overactive bladder.  Sometimes, less commonly (<1-2%), this may be caused by the loss of sphincteric muscle used in controlling urination.

Did you know? Dr. Shteynshlyuger has performed over 1,000 surgeries and procedures for enlarged prostate, including Rezum, Urolift, prostate enucleation, laser ablation of the prostate, and TURP.  He performs over 100 prostate procedures a year.
