Rezum after Proton Therapy, Brachytherapy or IMRT for Prostate Cancer

Prostate cancer affects about 15% of men during their lifetime. About half the men with prostate cancer who require treatment for the malignancy receive radiation therapy.

Radiation therapy treatment options for prostate cancer include proton therapy, IMRT, and brachytherapy. All radiation treatments for prostate cancer can cause urinary symptoms due to underlying BPH as well as prostate swelling and edema induced by radiation therapy.

Common urinary symptoms after radiation therapy include slow urine stream, frequent urination, a sensation of incomplete bladder emptying, difficulty starting and stopping urination, and frequent urge to urinate.

While some of the symptoms may be related to radiation cystitis, frequently in men, symptoms of enlarged prostate contribute to the urinary problems experienced after radiation therapy for the treatment of prostate cancer.

Does Rezum Therapy for Enlarged Prostate Affect the Success of Radiation Therapy for Prostate Cancer?

Rezum therapy causes apoptosis (cell death) of the prostate tissue adjacent to the urethra. As a result, Rezum therapy will not affect the success of radiation therapy for the treatment of prostate cancer.  Rezum therapy does not interfere with the effectiveness of radiation therapy for the treatment of prostate cancer after radiation therapy.

Is There an Increased Risk of UTI or Bleeding After Treatment with Rezum?

Adverse events such as UTIs, dysuria (pain with urination), hematuria (blood in the urine), and urgency may occur after Rezum therapy, but these are usually minor and resolve within a few weeks [1]. Men who are treated for prostate cancer with radiation therapy may have more friable prostatic tissue and may experience more prolonged bleeding after treatment with Rezum therapy. Usually, the bleeding is minor and self-limited.

Rezum Therapy for Enlarged Prostate After Radiation Therapy: What to Expect

Usually, Rezum therapy can be performed in our office under local anesthesia. There is no special preparation necessary. You may eat and drink normally before and after the procedure. Most men require a temporary urinary catheter to be left in place after the procedure.

Other Treatment Options Available for Men with Enlarged Prostate After Prior Radiation Therapy

Medications such as Flomax and Uroxatral are effective for treating symptoms of enlarged prostate in most men who have had prior proton therapy, IMRT, or brachytherapy for prostate cancer.

UroLift procedure can also be effective in patients who were previously treated with radiation therapy for enlarged prostate. Patients who previously had permanently implanted prostate brachytherapy seeds may also be treated with Urolift procedure, but there is a concern that a number of Urolift stents may be deployed, but will not position in the right spot if they encounter a brachytherapy seed in their way. This does not predispose the patient to any risks but can increase the cost of the treatment significantly.

Other treatment options for enlarged prostate and urinary retention after radiation therapy include bipolar TURP, laser enucleation of the prostate, HOLEP procedure among others.
