What is Holmium Laser Ablation of Prostate (HoLAP)?
Holmium laser ablation of prostate refers to a procedure that uses holmium laser to ablate, destroy or evaporate prostate tissue that is obstructing urinary flow. Men with symptoms such as a slow urinary stream, urinary urgency, frequent urination, urinary retention and urination at night can benefit from holmium laser ablation of the prostate.
The benefits of holmium laser is its safety and precise control over energy delivery to the prostate tissue. Holmium lasers while effective at prostate tissue ablation, are not as predictably effective as 2nd and 3rd generation lasers used for laser prostatectomy as the efficacy Holmium laser is dependent on the tissue characteristic (density) of prostatic tissue. Holmium laser is still very popular for laser enucleation of the prostate.
What is Holmium Laser Enucleation of Prostate (HoLEP)?
Holmium laser enucleation of prostate refers to the removal of obstructing prostatic tissue from inside of the urethra using holmium laser to cut out the inner part of the prostate. At the end of the HoLEP procedure, the chunks of prostate tissue are then cut into smaller pieces (typically morcellated) and removed. Tissue can be evaluated by a pathologist to look for evidence of abnormalities including prostate cancer.
How Does Holmium Laser Ablation of Prostate Compare to TURP?
Holmium laser ablation of the prostate is an effective treatment option. Because the effect of holmium laser on prostate tissue is variable, it may provide have a very good outcome in some patients, but not others. Experienced urologists are able to achieve a more complete removal of the prostate with TURP more predictably than with HoLAP. This is one of the reasons why HoLEP is gaining popularity over HoLAP. The HoLEP procedure is more reproducible and less variable and can achieve results equivalent to TURP. Other alternatives to HoLAP including Greenlight laser and Thulium laser have emerged.
