Do Men Ejaculate Normally after TURP Procedure?

Most prostate surgery procedures to remove obstructing prostate tissue including TURP and bipolar TURP lead to lack of ejaculation or unejaculation, sometimes called ‘retrograde ejaculation”. Unejaculation is a very common side effect of prostate surgery affecting more than 80% of patients who undergo TURP procedure.  Unejaculation after TURP is irreversible.

Other treatment options for enlarged prostate such as laser ablation with holmium and Greenlight laser and enucleation also cause similar side effects.

Does Bipolar TURP Improve Ejaculation Function in Men?

There is no difference in reported frequency of an ejaculation in men who undergo traditional unipolar TURP compared to those who undergo bipolar TURP for treatment of enlarged prostate and urinary retention.

Can Ejaculation Function be Preserved in Men who Undergo TURP Procedure?

For men who desire to preserve ejaculatory function, an attempt at ejaculation-preserving TURP procedure can be made by limiting removal of prostate tissue close to verumontanum.  Studies show that in as many as 50-70% of men, ejaculation may be preserved after TURP procedure aimed at preserving ejaculatory function. 

The downside of ejaculation-preserving TURP procedure is that it is a partial TURP and not a complete TURP procedure. As a result, the full benefit of TURP procedure for treatment of urinary obstruction may not be achieved: improvement in urinary symptoms may be less than what can be achieved with a complete resection of the prostate (complete TURP).

For men who understand the risks that attempt at ejaculation-preserving TURP may not always be successful and that improvement in symptoms may be less than optimal, the procedure can be performed by our experienced urologists at New York Urology Specialists.
