Do Men Have Good Erections after TURP Procedure?

Most men tend to have similar erections before and after the TURP procedure when it is performed at New York Urology Specialists by our experienced urologists.  Some studies report worsening erectile dysfunction in men after the TURP procedure.  However, this is controversial and may be related to the surgical technique, and the amount of coagulation and vaporization performed which generates a significant amount of heat.  We try to avoid coagulation and vaporization during TURP surgery. 

Most commonly we perform laser surgery for the treatment of large prostate rather than TURP.  Likewise, bipolar TURP produces less heat dispersion which limits damage to the surrounding tissues.  Lasers generate a very focused beam which limits potential head-induced damage to surrounding nerves.

In the age group of men who would typically undergo TURP procedure, who may not be sexually active for a few months after TURP, natural aging may also be a cause of erectile dysfunction.  In other words, some men would have developed erectile dysfunction or worsening erections even if they did not have TURP surgery.

However, it is not controversial that transurethral procedures can cause scarring and penile shortening. 

To minimize sexual side effects from prostate surgery, we evaluate each patient individually and advise the treatment approach that fits the needs and desires of the patient.

Does Bipolar TURP Improve Erection Hardness Function in Men?

There is no significant difference in terms of sexual function and erectile function between men who undergo traditional TURP vs bipolar TURP.  Treatment with bipolar TURP does not typically lead to improved erection function.

Can Erection Problems be Treated in Men who Undergo TURP Procedure?

Men who experienced erection problems before the TURP procedure or who have started to experience erectile dysfunction after TURP can be successfully treated with medication such as Viagra (sildenafil) and Cialis (tadalafil).  If pills do not work, an inflatable penile implant offers an effective treatment option with a high satisfaction rate.  Penile injections of Trimix or Caverject can also be used to achieve adequate erections for intercourse.

What Options are Available for Men with Good Erections who Wish to Preserve Erections?

Men with enlarged prostate symptoms and good erections who are worried about the effect of TURP on erections can consider procedures such as Urolift or Rezum which do not cause any sexual side effects; the trade-off is that they are less effective than TURP in improving urinary function.  Laser enucleation of the prostate does not seem to have associated erectile dysfunction and maybe a good option for men with large prostates.  

Aquablation is another procedure that is more effective than TURP but is associated with significantly fewer sexual side effects than TURP.  Because it does not generate significant heat, it is unlikely to have a deleterious effect on erections.
